Dosing Considerations – Should tend as Follows:

Pregnancy Termination:

Mifeprex: Indicated for the medical termination of intrauterine pregnancy through 70 days gestation together with misoprostol
Day 1: 200 mg of mifepristone orally as one dose under physician supervision
Days 2-3: 800 mcg of misoprostol within the cheek (buccally) once as one dose; must administer a minimum of 24 hours and a maximum of 48 hours following mifepristone dose on day 1 (see Administration)
Days 7-14
Must return for a follow-up visit to verify complete termination has occurred by medical record , clinical examination, hCG testing, or ultrasonographic scan
If complete expulsion has not occurred, but the pregnancy isn’t ongoing, women could also be treated with another dose of misoprostol 800 mcg within the cheek (buccally) with follow-up in approximately 7 days
Lack of bleeding following treatment usually indicates failure; however, prolonged or heavy bleeding isn’t proof of an entire abortion
Surgical evacuation is suggested to manage ongoing pregnancies after medical abortion

Dosing Considerations

Pregnancy is dated from the primary day of the last menstrual period
Duration of pregnancy could also be determined from menstrual history and clinical examination
Assess the pregnancy by ultrasonographic scan if the duration of pregnancy is uncertain or if extrauterine pregnancy is suspected
Remove any IUD (IUD) before treatment
Because most girls will expel the pregnancy within 2-24 hours of taking misoprostol, ask the patient an appropriate location for her to be when she takes the misoprostol, taking under consideration that expulsion could begin within 2 hours of administration

Cushing Syndrome

Korlym: Indicated to regulate hyperglycemia secondary to hypercortisolism in adults with endogenous Cushing syndrome who have type 2 DM or glucose intolerance and have failed surgery or aren’t candidates for surgery
300 mg orally once/day initially; may dose to a maximum of 1200 mg/day, but shouldn’t exceed 20 mg/kg/day
An increase in dose shouldn’t occur more frequently than every 2-4 weeks and will be supported by an assessment of glucose control, anti-diabetic medication requirements, insulin levels, and psychiatric symptoms

Dosage Modifications (Korlym)

Coadministration with strong CYP3A inhibitors: to not exceed 300 mg/day

Renal impairment: No change in initial dose; limit maximum dose to 600 mg orally once/day

Hepatic impairment

Mild-to-moderate: No change in initial dose; limit maximum dose to 600 mg orally once/day
Severe: don’t use
Pediatric: Safety and efficacy not established
